Assisting participants with travel transportation to and from destinations.
Participating in Community, Social and Civic Activities
Assisting participants to attend areas in the community for example. shopping trips, appointments and leisure activities.
Innovative Community Participation
Assisting participants to access mainstream activities such as football games, fishing, bowling.
Group and Centre Based Activities
Assisting participants to access all types of activities in a big group or centre-based setting, for example soccer, basketball, swimming, fitness, camping.
Developing Daily Living and Life Skills
Assisting participants to develop and maintain important life skills such as learning about public transport, resumes, taxes, saving skills, social skills, physical activity skill.
Coordinating or Managing Life Stages, Transitions and Supports
Assists participant’s capacity to transition from one stage to another in life such as peer support, mentoring, mental health recovery, coaching, support coordinations.
Daily Life Tasks in a Group or Shared Living Arrangement
Assist participants with home and personal care within a specialised disability accommodation or within shared living arrangement
Community Nursing Care
•Daily Personal Activities: Assists participants in receiving advanced care or training supplied by an EN or RN such as medications, wound care, 24/7 nursing care, tracheostomy management
•High Intensity Daily Personal Activities: Assists participants with complex daily personal activities such as bowel care, wound care, catheter care, tracheostomy management
