Feedback and Complaints Form

At Halo Health Support, we are committed to providing high-quality NDIS support and services. If you have any concerns or complaints about the services we provide, we encourage you to share your feedback. Please complete the form below, and we will ensure a response within 3 business days.

We are dedicated to protecting the confidentiality of all information provided during the complaints process. Any information shared will only be disclosed when required by law or when deemed appropriate based on the circumstances.

You have the option to submit your complaint anonymously. You can do so by:

  • Calling us at (02) 9188 6448 and requesting to file an anonymous complaint (no personal identification will be required).
  • Filling out the Feedback and Complaints Form but leaving your name and contact details blank.


Right to Access Advocates

You are entitled to seek support from a family member, friend, or independent advocate when making a complaint. If you wish to engage an advocate or representative, please inform us, and we will gladly assist in facilitating arrangements.


How to File a Complaint with the NDIS Commission

Complaints about services or supports can also be made directly to the NDIS Commission. You can file a complaint by:

  • Calling 1800 035 544 (free from landlines) or TTY 133 677. Interpreter services are available.
  • Using the National Relay Service and requesting 1800 035 544.
  • Visiting https://www.ndiscommission.gov.au/about/complaints to complete an online complaint form.

The NDIS Commission accepts complaints regarding:

  • Services or supports provided in an unsafe or disrespectful manner.
  • Services or supports that were not delivered to the required standard.

Complaints can also be made to the NDIS Commission on an anonymous basis.
